If you need to see the doctor, nurse or emergency department several times a year, you may be eligible to receive Care Plus.
It sounds special, and it is. Care Plus keeps health-care costs down for people with chronic health conditions, acute medical or mental health needs, or a terminal illness.
Care Plus also helps doctors, nurses, pharmacists, physiotherapists and people helping care for your health to work as a team to improve your quality of life.
Ask your doctor or nurse whether you, or someone you care about, may be eligible to enrol in the Care Plus Programme.
