Ranolf Leads the Way by Achieving all Quality Targets

Ranolf Medical Centre has become the first practice in the RAPHS region to reach all population and equity targets - no easy feat. 

At Ranolf, the team believe that targets are just the beginning.

"Our team spent a lot of time thinking about the reasons targets exist and what the point in them was," says Caerlie Picken, a Nurse Prescriber and the nursing team leader at Ranolf.

"From there, we worked together to find ways to address the underlying health needs of our community."

Delia takes control of her diabetes after DESMOND training

Delia Mckinnon was already keen to take control of her diabetes when she heard about two programmes that would help her do this.

The first was Green Prescriptions, the Sport Bay of Plenty programme that tailors exercise and healthy eating programmes for people who want to improve their lifestyles.

Through this, Delia also learned of DESMOND, the sessions regularly held at RAPHS which are specifically tailored to give people with Type 2 diabetes the information they need to take control of their health.

Green Prescriptions help people help themselves

Brenda Sommerville's health and fitness journey began 25 years ago, when her doctor informed her she was at risk of having diabetes.

"What is that?" she thought.

Lacking knowledge about the finer points of diet and exercise, and with fewer resources available than there are today, Brenda was unaware that she could change her lifestyle to combat the disease.

Charitable Trust brings Whanau Ora to Pacific Islands community

The concept of Whanau Ora has been warmly embraced in the RAPHS region, particularly among local iwi and the health providers who work with them.

However, our region also has 3500 Pasifika residents who have accessed various Whanau Ora services. One organisation's response to this, with the help of a team of hard-working people who are dedicated to improving the health and wellness of this community, is to offer its own Whanau Ora support for Pacific Island families.

We're talking about the Rotorua Pacific Islands Development Charitable Trust, which is managing a variety of projects in the RAPHS region.

Registered Nurse Joins RAPHS' Extended Care Support Team

Annabel Senior brings experience from a diverse range of locations to her new nursing role with RAPHS’ Extended Care Support Team.
 
Between her childhood in Foxton and moving to Rotorua three or four years ago, Annabel has worked “all over the place” in a variety of jobs mostly involving acute care.
 
These include time at Rotorua and Tauranga DHB’s, Middlemore and North Shore Hospital’s ICUs, and stints with agencies in rural areas in Queensland and New South Wales. It also included working in ophthalmology clinical trials as a clinical nurse research co-ordinator for the University of Sydney.
 
The diversity of these locations has taught Annabel that health care can be approached in different ways despite shared foundations.

RAPHS puts General Practice first with funding for new services

The RAPHS Board requested the development of further acute demand services, and how best to deliver additional FTE to support improved patient health outcomes.

Following FAS committee and clinical governance sign-off, the following initiatives have been approved by the Board and will commence from 1 April 2018.

To support these services, a RAPHS costing methodology has been established, in consultation with RAPHS member practices, to provide a robust costing approach based on time and cost in delivering primary care services.
