RAPHS comprehensive care team is operating out of Koiora building at 1185 Tutanekai Street, delivering an interdisciplinary health and wellbeing service with a focus on;

  • Reducing health inequity through improving access
  • Integrating with other health services and agencies
  • Providing responsive care delivery
  • Supporting enrolment into General Practice

Koiora service staff: are whānau centred; integrated, multidisciplinary and evidenced based; embracing of diversity and complexity; timely, accessible, responsive and culturally safe; committed to safety and quality; free of charge to the patient and time limited.

About us

Koiora provides an opportunity for RAPHS to focus service delivery to promote timely access to short-term, high-quality, culturally safe and appropriate care through a centrally located service centre integrated with local community providers including general practice.

Koiora service provision is hope-inspiring, strengths-driven, and collaborative and assumes people can improve their health and lead fulfilling lives and contribute meaningfully to their communities and society more broadly. Te Whare Tapa Wha is the preferred assessment framework to ensure that taha tinana, wairua, whanau and hinengaro (physical, spiritual, family and mental health) needs are addressed.

Services

Service Delivery

Services delivered through the Koiora service centre may be delivered by RAPHS, contracted RAPHS service providers or partners of RAPHS and will support the Rotorua Community with;

  • Free or low cost access to integrated and interdisciplinary comprehensive care services for eligible service users
  • Improved service access and health navigation support
  • Targeted service delivery to address identified unmet health need for high needs groups to complement and support existing community based services including general practice
  • Digital health access including telehealth support
